Although he's a little hard on the interns, other doctors, most patients, and generally everybody, it's only because he's pretty sure he knows everything.
Truth be told, Dr. Cox takes his job very seriously, and if he seems a bit abrasive at times, it's only because he cares. He doesn't want anyone to know it, but he probably cares too much. And that's why he gives the other doctors a hard time. He wants them to be as good a doctor as he - if not better. And so, somehow, amidst his incessant condescending ramblings, there's usually a good lesson... if you're man enough to handle it.
